    Complete the following questions to determine if you are currently eligible to make an appointment to receive the SARS-CoV-2 (COVID-19) vaccine at this time.

    This tool will help determine if you are eligible to receive a vaccine per current Washington State Department of Health authorized phases. If you are eligible, you may use this tool to self-schedule your first-dose appointment. If are not, you will need to return when you are eligible to schedule an appointment.

    MultiCare is committed to providing vaccine to eligible members of our community. Please note that appointments will be cancelled at the time of service in the event eligibility requirements are not met.

  • MultiCare is currently scheduling first dose vaccine appointments for eligible individuals in Phase 1A and Phase 1B, tiers 1-4

    To be able to schedule your vaccine appointment, you must be able to answer “yes” to at least one of these eligibility questions:

    • Are you 60 years of age or older?

    • Do you work in a health care setting or with medical patients?

    • Are you 50 or older and live in a multigenerational household; for example, a grandparent living with grandchildren.

    • Are you a K-12 school teacher or staff or a certified childcare worker?

    • Are you a child care worker, a licensed family home child care provider, or a family member living in the home of a licensed, in-home child care provider?

    • Are you a high-risk critical worker working in public transit or one of these congregate settings: agriculture; fishing vessel crews; food processing; grocery stores; corrections; prisons, jails or detention centers; restaurants; food services; construction or manufacturing?

    • Are you 16 years or older and are pregnant, have two or more co-morbidities or underlying conditions, or have a disability that puts you at high risk for severe COVID-19 illness?

    • Do you work as a first responder? For example, an EMT, fire fighter or law enforcement?

    • Do you live, work or volunteer at certain congregate living settings such as correctional facilities; group homes for people with disabilities; or in settings where people experiencing homelessness live or access services?

    The Puget Sound includes Auburn, Puyallup and Tacoma vaccine clinic locations.

    We currently only offer the Moderna vaccine in our Spokane vaccine clinic and are only able to accommodate patients 18+ at this time. We can accommodate patients ages 16-17 in our Puget Sound vaccine clinic locations with the Pfizer vaccine.
